Delegate Survey Form completed by Otto E. Huiswoud for the 6th National Convention of the Workers (Communist) Party of America. Over 180 voting delegates, alternates, fraternal delegates, and guests to the 6th National Convention of the Communist Party of America completed survey forms for the party archives, detailing their age, address, citizenship status, social origins, previous organizational memberships, arrest history, and union affiliation. This is the survey form completed by Otto E. Huiswoud, charter member of the Communist Party and highest ranking black cadre in the American Communist movement. This document is an important source of biographical information on this pioneer black Communist leader. Original document resides in the Comintern Archive, RGASPI f. 515, op. 1, d. 1598, l. 96.
